Requesting Records
You have the right to request copies of your health care records maintained by this practice.
To request records, please submit a written request by email to [email protected]. Requests should include your full name, date of birth, the records being requested, and your preferred method of delivery. Additional identity verification may be required before records are released.
Records requests will be processed in accordance with applicable Texas and federal law. Fees may apply as permitted by law.
